Greater Noida, Aug 24: Doctors at Yatharth Super Speciality Hospital, Omega-1, Greater Noida, successfully managed a complex case of right occipital lobar hematoma (brain hemorrhage) in a 56-year-old man through timely diagnosis, advanced neuroimaging, multidisciplinary care and conservative neurological management, enabling the patient to recover without surgical intervention.
The patient presented to the OPD with neurological complaints and underwent immediate evaluation after reaching the hospital. During his hospital stay, on the sixth day, he developed sudden visual disturbances, including difficulty seeing objects and an enlarged blind spot. The patient was advised an immediate non-contrast CT scan, which revealed a right occipital lobar hematoma, prompting immediate neurological intervention and further evaluation.
A subsequent MRI of the brain with MR angiography confirmed an acute-to-subacute right occipital lobar hematoma with surrounding perilesional oedema. Importantly, no vascular abnormalities were detected on MR angiography, ruling out underlying vascular malformations. The patient was managed conservatively with anti-oedema therapy, close neurological monitoring and supportive treatment.
The patient was a known case of hypertension, chronic kidney disease and gastritis, requiring coordinated management from neurology, nephrology and gastroenterology teams. Blood pressure was gradually optimised, while nephrology experts deferred kidney biopsy due to the recent intracerebral haemorrhage and associated bleeding risk. He was also evaluated for gastritis, diagnosed with Helicobacter pylori infection and started on appropriate medical treatment.
Throughout the hospital stay, the patient remained under continuous observation. He responded well to intravenous antibiotics, analgesics, antiemetics and supportive care. His neurological status improved steadily, and he was discharged in a hemodynamically and neurologically stable condition on oral medications with appropriate follow-up advice.
