FORT WORTH, Texas, March 24 — Palm Primary Care (“Palm”), part of the MBF Healthcare Partners II, L.P. (“MBF”) portfolio and affiliated with Florida-based Palm Medical Centers, has acquired two primary care clinics in Azle, Texas, from Texas Health Physicians Group. This acquisition expands Palm’s Texas footprint to nine clinics in Tarrant and Dallas Counties and further strengthens the company’s commitment to expanding access to high-quality primary care to North Texas residents. The transaction reflects Palm’s continued growth strategy in the region and its commitment to expanding access to high-quality primary care through targeted acquisitions.
“This expansion continues access to high-quality primary care in Azle while preserving what patients value most: the relationships they’ve built with their care teams,” said Fowad Choudhry, CEO of Palm Primary Care. “We’re proud to welcome these clinics into Palm, keep care local, and support the physicians who have served the greater Azle community for decades.”
Both clinics remain in their current locations, maintain the same office hours, and continue serving patients with the clinicians they know. Three physicians and five advanced practice providers transitioned to Palm Primary Care as part of the acquisition.
The two locations will operate as Palm Primary Care – Azle Central, located at 141 Industrial Avenue, Azle, TX 76020 (phone: (817) 444-3231), and Palm Primary Care – Azle Parkway, located at 909 Southeast Pkwy, Suite 105, Azle, TX 76020 (phone: (817) 238-0735).
“Patients will continue to receive primary care that is consistent, personal, and focused on long-term health,” said Pedro Daniel Gonzalez, M.D., Medical Director for Palm Primary Care – Azle Central. “This transition allows us to continue caring for families in Azle while strengthening the resources that support preventive care and chronic condition management.”
Palm Primary Care works with a broad range of insurance plans, including Medicare Advantage, and expects most patients will see no change to their current coverage. Patients can contact their clinic directly to schedule appointments or ask questions.
